Student Registration and Hold Harmless Form

We will confirm there is space available in the class of your choice, and send you an e-mail confirmation along with additional instructions for mailing your registration fee. Once your $45 registration fee is received, you will recieve an e-mail confirmation that your space has been officially held. The registration fee is non-refundable and covers the cost of your workbook and/or classroom handouts. Tuition is due on the first day of class each month.

HOLD HARMLESS AGREEMENT

I understand the sewing instructor will do her best to prevent an accident from occurring, however, I, the undersigned, agree to hold Kim Cintron harmless for any accident that may occur to my child while attending or in the process of attending sewing class, and while attending sewing activities held by AshTal Fashions Sewing Studio.
